         at least up to the 3rd grading period
         grade requirement (CAS/CoEd/CoE Courses): GPA of 85 and above and no grade below 80 in any subject.

Certification of Good Moral Character

Two Copies of Recent 2x2 Colored Picture with white background and nametag

Testing Fee of Php 200.00
