Undergraduate Programs Requirements
UNDERGRADUATE
General Requirements
Must pass the entrance examination and Personal Interview and comply with the following basic requirements:
- Photocopy of PSA-issued Birth Certificate
- Medical Certificate from the Health Doctor
- Chest X-ray, C and Drug Test Results (common to all courses)
- Stool Examination and Hepa (for BSHM applicants only)
- Complete Medical Examination Package (for BS Mar E) from the accredited clinic of MARINA, DOH and POEA
- Original copy of Certificate of Good Moral Character
- Three (3) copies 2”x 2” recent colored picture with white background and name tag
- One (1) copy of 1”x1” recent colored picture with white background and name tag
- One (1) Long Brown Envelope (14.5” x 9.8”)
Specific Requirements
Senior High School
- Original Copy of Form 138A (Report Card)
ALS Passer
- Original copy of the Certificate of Eligibility and Original copy of the Certificate of Rating
Transfer student
- Transcript of Records (Informative copy)
- Honorable Dismissal (Certificate of Transfer Credential)
Returnee /Shiftee
- Grade Slip for the Last Semester Attended
Entrance Examination Requirements
Note: There is a screening committee to screen the requirements of the applicants for the incoming Freshmen Student, Transferee, Returnee and Shiftee before taking the Entrance Examination.
General Requirements
- Photocopy of Certificate of Good Moral Character signed by the principal or guidance counselor
- Two (2) copies 2”x 2” and 1x1 (1 copy) recent colored picture with white background and name tag
- PSA photocopy
- One (1) Long brown envelope
- Testing Fee is Free
Specific Requirements
Senior High school Graduate / High School Graduate
- Photocopy of Form 138A (Report Card)
ALS Passer
- Photocopy of the Certificate of Eligibility
- Certificate of Rating
Transfer student
- Transcript of Records (Informative copy)
- Certificate of Transfer Credential (Honorable Dismissal)
Returnee /Shiftee
- Grade Slip for the Last Semester Attended
Note: Bring a pencil for the entrance exam. Applicants with incomplete requirements will not be allowed to take the entrance exam.
Grade Requirements
Board Courses
- Grade Point Average at least 85
- English, Math, Science & Areas of Specialization—Individual Final ratings will be 85 or higher
Non-board Courses
- Open admission and selective retention
