First year college students
- Photocopy of Grade 11 & 12 Report Cards;
- Photocopy of PSA/NSO Birth Certificate;
- Photocopy of Certificate of Good Moral Character;
- Two (2) pcs. 2x2 picture (w/ nametag and white background);
- One (1) pc. Long, white folder.

Alternative Learning System (ALS Passers)
- Photocopy of Certification of Passing;
