New Students/ Transferees/Non-Alumni (DPE & MAEd)
- Completed a Bachelor’s Degree
- Scanned copy of TOR – Satisfactory Scholastic Record (with school seal & date of graduation)
- Scanned copy of the Honorable Dismissal (to be processed from the previous school attended)
- Scanned copy of PSA Birth Certificate (Marriage Certificate for female applicant)
- Transfer of Credentials
- 2x2 picture with a name tag
- E-Signature
