SPECIAL PROGRAMS
Specific Requirements
Certificate of Professional Education (CPE) (Transferees)
- Honorable Dismissal or Transfer Credentials;
- Birth Certificate (PSA Authenticated) – photocopy;
- Marriage Certificate (for married females) – photocopy;
- Medical Requirements (CBC, Chest X-ray, Drug Test, Medical Certificate) – original copy;
- Recent 2x2 colored ID picture with a white background and a name tag (2 copies);
- Official Receipt of the Testing Fee;
- Test Result;
- Long, brown envelope (expanded).

- Medical Requirements (CBC, Chest X-ray, Drug Test, Medical Certificate) – original copy;
- Marriage Certificate (for married females) – photocopy;
- Recent 2x2 colored ID picture with a white background and a name tag;
- Long, brown envelope (expanded).
Note: Other requirement/s may be added as necessary to any of the programs offered.
