- Honorable Dismissal (previous school) – original copy
- Certificate of Transfer Credentials
- Granted Transcript of Records (TOR) – original copy
- Birth Certificate
- Marriage Certificate (for married females) – photocopy
- Master’s Thesis Book (for Doctorate program enrolees) – 1 copy
- Medical Certificate
- Chest X-ray
- CBC
- Drug Test
- Official Receipt of Testing Fee
