SPECIAL PROGRAMS
Certificate of Professional Education (CPE)/Diploma of Professional Education (DPE) (for Transferees)
- Honorable Dismissal or Transfer Credentials
- Birth Certificate (PSA Authenticated) – photocopy
- Marriage Certificate (for married females) – photocopy
- Medical Requirements (CBC, Chest X-ray, Drug Test, Medical Certificate) – original copy
- Recent 2x2 colored ID picture with white background and name tag (2 copies)
- Official Receipt of the Testing Fee
- Test Result
- Long, brown envelope (expanded)
